Fredonia Middle School

For your convenience the Middle School has developed a form to be completed and submitted whenever your child is late to school, absent or needs to leave school early for an appointment. Checking the boxes on this form and signing the bottom of the document will save you the trouble of writing up an entire note. The form also makes it easier for our clerks to accurately record your child's attendance. Pads of these forms are available in the FMS office and the health office as well as on this site.  You can find a link to the form in the Middle School drop-down menu at the top of the page, as well as in the "Information for MS Parents" menu on the left-hand side.  A copy is also attached to this story.

Dear Parents,
This letter is to inform you that there are students in the 5th grade who have severe peanut/nut allergies. Strict avoidance of peanut/nut products is the only way to prevent a life threatening allergic reaction.
