The Fredonia Central School District is required to conduct an annual census to identify disabled children six to twenty-one years of age. Parents or guardians who have a disabled child or disabled children who are not enrolled in the District’s special education program should complete and return the form below to initiate registration of their children. Parents or guardians with questions should call the Chairperson of the Committee on Special Education (679-1581 ext. 2736) prior to completing the form.
